Adjustable gastric banding

Adjustable gastric banding is a modern obesity treatment method. It involves surgical placement of a special band around the top portion of the stomach intended to narrow the entry to the stomach. Receptors that transmit signals about satiety to the brain are located at the top portion of the stomach which is separated by a band. The volume of this stomach part is about 20 ml. So, to feel satiety a person needs to eat just one tablespoon of food.

Procedure of adjustable gastric banding surgery:

A band is placed via laparoscopic surgery. In the course of the procedure the abdominal wall is pierced and all surgical manipulations are performed using a laparoscope and other instruments. Laparoscopy reduces recovery time significantly.

During the procedure a band is not filled with fluid. Only the placement of a band is fulfilled. In 1-2 months an X-ray examination is carried out. If a band is well-secured in the stomach tissues it will be filled. A need may arise to adjust the filling level several times to achieve the best result in loosing weight.

Food chewing technique is an important part of a band placing method. After such procedure solid and protein-rich food should be primarily consumed. All fruits and vegetables should be chewed well. At the same time, food should not be puréed using a blander.

Preparation to adjustable gastric banding surgery:

Preparation includes a comprehensive examination of a patient. Blood and urine samples are provided for analyzing. A patient undergoes ECG, X-ray and ultrasound examination of the abdominal cavity organs. Blood is also tested to reveal hidden infections (AIDS, hepatitis, syphilis, etc.). The blood clotting ability and Rh factor are determined. It is obligatory to have a consultation with a dietitian and anesthesiologist.

Sticking to a special diet during the preparation stage is extremely important. Its purpose is to reduce the amount of fat in the liver cells effectively. If the liver is enlarged due to severe obesity a surgeon is unable to perform a band placing procedure via laparoscopic surgery. In this case, open surgery is required. This is because a part of enlarged liver covers the esophagus and stomach. If a patient sticks to the recommended diet for a week the excess liver fat can be removed without any effort.

A week before surgery a patient must stop taking any medicines affecting the blood clotting ability and NSAIDs.

No eating and drinking is allowed 8 hours before surgery. During this period the intestines are purged using enema. Surgery is started in the morning on an empty stomach.

Surgery/procedure duration: 2 hours

Anesthesia: general anesthesia

Postoperative period after adjustable gastric banding surgery:

A patient is transferred to a ward immediately after surgery. In case a band has been placed via laparoscopic surgery a patient stays at a ward for 3 days, if open method has been used this period is extended to 7 days. Eating is allowed on the next day if no problems with swallowing are experienced. Drinking is permitted in 5 hours after surgery.

In the evening a patient can get out of bed carefully. In is advised to increase physical activity gradually to avoid blood stagnation in the legs.

Painkillers are prescribed if a patient feels pain. Bacterial therapy can be introduced to prevent infections. All scars are treated with antiseptic solutions daily. They must not get wet during a week after surgery.

If nausea and vomiting are experienced it is necessary to take medicines preventing them. After discharging from the clinic a patient should stick to the prescribed diet. During the first two weeks only liquid food must be consumed. Then solid food is added which must be chewed properly.

The intensity of physical activities is increased gradually. Lifting of heavy weights is not allowed for a month following the procedure. Also, it is forbidden to get out of bed using the abdominal muscles.

The expected effect can be observed in a few days after a band has been filled and adjusted for the first time. In order to achieve a sustainable result it is important to follow the recommendations of Deva Clinique doctor.
